** For 2010-11 Kindergarten Enrollment Forms - Please click here Families are now able to enter information directly on the enrollment forms listed below. (click the highlight fields box on the right side of the page to help you see what lines need to be entered) Once you have entered your student information on the form, print the pages and take them to your resident school to enroll your children. Please bring your proof of residency, children's original birth certificate or passport, immunization records and any other important information related to your student. Current Kindergarten and 1st Grade The fillable enrollment forms can be found in the Enrollment Binder Grades K-1 PDF file (click the highlight fields box on the right side of the page to help you see what lines need to be entered). You will also need to print the Pupil Health Exam for School Entry form and take to your physician.
